Common mistakes when completing the CRB application form
Applicant does not use BLOCK CAPITALS when completing the application form
Blue ink is used Only black ink is to be used
Tippex Errors should be neatly crossed through. Tippex or other correcting fluid must not be used
Five year address history There must be no gaps and/or overlapping in address histories
Middle name missing When an applicant submits information such as a driving licence number and it indicates that a middle name exists, but this has not been given in Section A, Line 3 of the Application Form, the CRB will reject and return the form
Gender is missing (Section A Line 11) Gender must be crossed. If there is a problem with stating gender, please contact the Admissions Office, crb@ucpmarjon.ac.uk
Surname at birth missing If an applicant crosses Ms or Mrs in Section A, Line 1, they must enter surname at birth in Section C, Line 20. The surname at birth must be provided, even if it is the same as in Section A
Place of birth is missing (Section C, Lines 28/29 This must be completed even if you were born in another country
Signature missing You should make sure that you sign (and date) the form at Section H, Lines 68
Visual identity must be verified We must verify the identity of all CRB applicants. This can be achieved by the identification documents being presented at interviews where appropriate, at the Registry Office, UCP Marjon or by using the photo identification form.
